COLE Connect Online Patient Health Tool Launch


Cole Memorial Medical Group is pleased to unveil COLE Connect, a new online health tool which will be available to all ambulatory patients at Cole’s medical offices starting September 3, 2014. A launch of COLE Connect for the Hospital’s inpatients will follow on October 1.

“This free, interactive tool can empower patients to take a more active role in their care by providing easy, secure access to their health information and online communication with Cole Memorial’s staff,” said Ed Pitchford, president and CEO of Cole Memorial. “We encourage providers to enroll in Cole Connect and to take advantage of this tool and let us know how their experience has gone.”

Once patients click the COLE Connect enrollment button located on Cole Memorial’s website, only a few pieces of information must be entered before using the portal, including:

· A Cole medical record number that is accessible from their provider’s office; by email at ColeConnect@charlescolehospital.com or is printed on most patient documents

· Personal email address

· Name and birth date

COLE Connect gives patients secure, instant access to their health information, including laboratory and radiology results, medications, allergies, visit history, discharge/aftercare information, and more. Additionally, COLE Connect allows patients to view their appointments online. Patients can also access their billing statements and outstanding balances and submit electronic payments.

Cole Memorial Medical Group’s Medical Director Marlene Wust-Smith, MD said, “All of our patients from the primary care providers’ and specialists’ offices should enroll as soon possible in COLE Connect, as it presents a valuable opportunity for them and their families to actively participate in their care, which contributes to better health.” (READ COMPLETE MEMO HERE.)
